The ACCCOS 2020 Relief Grant

In light of the recent pandemic, ACCCOS has not been able to hold in person General Membership Meetings this year which has created a small surplus of funds. In an effort to give back to our local community of doctors, our board has voted to establish a relief grant for our optometric community.

Grant Criteria
The purpose of this grant is to provide assistance to local optometrists who may be struggling financially due to recent challenges. These challenges include but are not limited to those resulting from COVID-19, civil unrest, and theft.

Eligible Recipients
Any optometrist practicing or residing in Alameda or Contra Costa Counties may apply who can demonstrate or show documentation that they have experienced a recent hardship (within the 2020 calendar year).

Evaluation Procedure
The applications will be reviewed by the ACCCOS Board of Trustees. Three candidates will be awarded a $1000 grant each based on severity of demonstrated need. We will also factor in if a candidate has received any other grants, assistance, or aid. Grant applications must be submitted by September 30, 2020. Recipients will be notified by October 15, 2020. Recipients will be required to fill out and submit a W9 form to receive their payment via check. You are not required to be a member of ACCCOS to apply for this award. Submissions will be kept anonymous.
